About the Youth Transitional Care Program
The Youth Transitional Care Program (YTCP) is a 14 week program for young people aged 12–18 years in the Greater Newcastle area who are having a hard time with their mental health. This support program will give you the chance to talk, set up a recovery plan and learn to live your best life. You may receive further weeks of service if a high level of psychosocial support need persists.
You can expect one-on-one support from a truly caring recovery worker for the journey of the program. We’ll work with you (and your family or other supports) to create a recovery plan that works for you and connect you to other supports in the community if you need them.
We’ll give you tips on looking after yourself and your wellbeing, so you can develop the confidence to stay connected in school or work, and with family and friends. The recovery worker will support you however you need them to. Whatever your situation, background, cultural needs, or individual preferences, you’ll always be respected and heard.
The Youth Transitional Care Program is for anyone who:
- is aged between 12 and 18 years of age
- lives within one hour of Greater Newcastle
- needs more support with their mental health challenges
- finds it hard to cope with day-to-day life, like attending school, socialising or finding a job.
- is not currently receiving adequate NDIS support or another similar support.
If you are under 14 years of age you will need a parent, carer or legal guardian to help you receive our support. If you are a parent or guardian of a child under 14 years of age who needs our services, we advise that you attend the first meeting with us.
How to access this service
The Youth Transitional Care Program is a free service provided by Wellways. It is funded by The Primary Health Network (PHN) and delivered in collaboration with Hunter New England Local Health District.
