About YouThrive

The YouThrive team works alongside young people to build confidence, set meaningful goals, and create positive change in their lives.

Support is provided to help young people stay connected to family, friends and their community, while encouraging participation in education, training and employment. The program focuses on building independence, improving wellbeing and reducing the need for hospital stays through timely, tailored support.

Families and carers are also supported to strengthen positive relationships with their young person and develop a better understanding of mental health.

YouThrive supports people to:

  • live more independently in the community
  • better understand and manage their mental health and wellbeing
  • build confidence, resilience, and social connections
  • develop a positive sense of self and identity
  • manage everyday tasks and gain practical living skills
  • access education, training, or employment opportunities
  • find and maintain stable housing
  • get involved in community activities and hobbies
  • strengthen relationships with family and friends
  • develop strategies to manage safety

Accessing YouThrive

A referral is required to access YouThrive and must be made by a clinical care coordinator.

Eligibility

A young person may be eligible for YouThrive if they:

  • are aged between 15 and 24 years (inclusive)
  • are receiving clinical support from a Local Health District community mental health service have, or are at risk of developing, a significant functional disability
  • have provided informed consent to participate in the program
  • In some circumstances, young people who do not meet all eligibility criteria may still be considered where the program is likely to benefit them, and no other suitable supports are available.

All referrals are reviewed and considered based on eligibility. Eligible young people will be invited to meet with the team to learn more about the program, identify areas they would like to work on, and discuss how support can help them achieve their goals.
