When you join the Enhanced Adult Community Living Support Service, we will work with you to create a unique program to help you build a good life.

What is Enhanced Adult Community Living Support Service?

Enhanced Adult Community Living Support Service is an outreach service, which means we can work with you in your home or in the community.

We work with you to identify areas in your life where you would like to make positive change and then provide you with the support to develop strategies and access resources to achieve your goals. This process is driven by you and, where appropriate, also developed in partnership with your family and other services you might be accessing.

How does Enhanced Adult Community Living Support Service help?

We work with you to:

  • manage your mental health and wellbeing
  • live independently in the community
  • manage everyday tasks and improve living skills
  • participate in community life
  • improve resilience and social connections
  • access employment, education or training
  • sustain accommodation

How to access this service

To access Enhanced Adult Community Living Support Service you can self-refer or clinicians/services can refer on your behalf with your consent.
If you are eligible, please download the referral form below.

To be eligible for this service you must:

  • Be aged 16 years or older
  • Have a diagnosis of a mental health issue that is impacting on the person’s ability to live independently
    OR
    a) In the case of a young person, where a formal diagnosis is absent, eligibility will include functional impairment identified by a mental health professional
    b) be of Aboriginal descent, experiencing a social and emotional wellbeing concern. A diagnosable mental health condition is not required to receive support.
    c) in the case of refugee and asylum seekers, experiencing psychological distress, mental ill health and functional impairment. A diagnosable mental health condition is not required to receive support.
  • Receive ongoing support from or be eligible for support from Illawarra Shoalhaven LHD Mental Health Service, Murrumbidgee primary health network or another community-based clinical support willing to collaborate with Wellways. eg. Psychologist, Psychiatrist, your local health district Mental Health Primary Clinician, GP
  • Have provided informed consent to participate in the program.
