About Connected Care
Connected Care – Mental Health, funded by the Australian Government through Primary Health Tasmania (Tasmania PHN), will provide free support for adults experiencing mild, moderate, or severe and complex mental health concerns.
The service will be available across the state, with support offered in person, by phone and online. It will include urban hubs, as well as outreach to people living in rural and remote areas.
Wellways will lead delivery of the new service in partnership with Stride Mental Health and Clarity Health Care.
The service will be established over the coming months, with full implementation by 31 January 2027.
Primary Health Tasmania is overseeing the establishment of Connected Care – Mental Health. For information about the service before it opens, visit the Primary Health Tasmania website.
