Well Connected is a psychosocial support program for people who experience severe mental illness but who are not getting assistance from the National Disability Insurance Scheme (NDIS).
Well Connected helps to build the capacity and stability of people needing supports and services in many areas of their lives. This can include family relationships, managing daily living needs, budgeting, housing, health, education, work and other personal goals.
It is a psychosocial support program providing a range of community-based mental health supports including outreach, peer support, and group based programs for people living with mental illness in the community and their families.
Well Connected promotes the importance of connection, wellbeing and community inclusion while also providing practical support to participants. The program seeks to provide short term support for people to pursue wellbeing and inclusion.
In addition, the program aims to work directly with community members and groups to establish welcoming and mutually supportive spaces for all people.

Well Connected is funded by the federal government through Primary Health Networks and delivered by Wellways Australia.
The program is funded in the Murrumbidgee region of New South Wales by the Murrumbidgee PHN.
