Pathways to Home is an assertive outreach program that is being delivered by Wellways in the local government area of Maroondah. It is part of the Victorian Government Homelessness and Rough Sleeping Action Plan and is a two-year pilot program.
The Pathways to Home program engages with people experiencing rough sleeping by supporting them to connect with their community and connect them in with the services they need. This includes assertive linkages to physical and mental health services, drug and alcohol services, legal, housing and other services, and providing advocacy to those services where needed.
To be eligible for referral to Pathways to Home, someone must be:
- experiencing rough sleeping
- located in the Maroondah region
- willing to accept support from a Wellways outreach worker
